It would interesting to know if this was something the center could prevent in the future (or mitigate some fo the risk) or an event that was truly "unexpected." A follow on question would be: What did the center do after the "dust settled" to get to the root cause, assess real impact and make process changes to avoid, change or adjust processes for the future.
